They age up automatically when a new game year starts at the beginning of each real life month.

I think foals shift up to yearlings in the middle of their first game year. If you create a foal after the middle of a month, I think it now begins life as a yearling, but that's a recent change Ammit has made.

When I first started playing, a game year was two months long on this server, but that changed a number of real life years ago.
