DHH tattoos do not appear on the horse. The star you saw would have been created by the stable owner and they placed it themselves, it could have just been their own stable tattoo, or they may have put stars on their DHH horses.

Tattoos are an upgraded feature though - you have to have at least a basic upgrade to create, add, or remove tattoos.

If you do upgrade, tattoos are created through the tattoo registry, which you can find in the left hand menu under the stable management header. They are placed/removed through the tattoo manager, which is a link at the bottom of the horse's control panel, located next to the tack manager link.
